dirname() is a slow function compared with __DIR__ and using
'/../'. Moodle has a large number of legacy files that are included
each time a page loads and is not able to use an autoloader as it is
functional code. This allows those required includes to perform as
best as possible in this situation.

These strings have already been copied from mod_quiz to qtype_numerical but
in the qtype_numerical plugin code the translations from mod_quiz were used.
Even though the strings were already present we copy the translations.

This was one of those innocent seeming issues where, once you start
digging, you find a mess. In this case, the code that is now in
question_wizard_form::add_hidden_fields used to exist in four different
places, in four inconsistent versions. This is now all nicely
re-factored, and that solves the problem.
Along the way, I found and fixed some wrong string references in
qtype_random, and stripped out some unnecessary &s in function
declarations.
There was a mistake in the text format upgrade in the question bank. The wrong conversions were performed, and the wrong arguments were passed to text_to_html in the conversions that were done.
Also, not all the calls to format_text had been updated to use the values in the new format columns.
I think this change fixes everything, but I have only had very limited time to test it. I am committing it anyway, because that seems to me to be the best way to maximise testing. I think that the new code is certainly better than the old code was.
From now on, all English strings use the new syntax. They are not
eval()'ed any more and the only valid placeholders are {$a} and
{$a->foobar}. No extra quotes escaping, dollar sign escaping and putting
double percent signs.
The modified files were exported from AMOS database repository in the
new syntax and were re-ordered by stringid. Standard GNU/GPL and PHPdoc
blocks are added. Where there was no copyright note so far, I added the
default one with Martin Dougiamas as the copyright holder.
